To access the database and enroll a patient, click here.

As the primary Registry physician, you must consent and then enroll and receive training on the Registry system before entering data into the LAL-D Registry. Following approval and training, you will be responsible for the patient data that are submitted to the Registry. You will also be able to add additional team members (Sub-investigator, Study Coordinator or Research Nurse) at your site, who will be directly involved in the care of your patients, and/or who will be entering data. You and your designees will comprise the Primary Care Team.
